Submitting a Chemical Analysis Order Form

2. PAYMENT

Obtain a purchase order number from your company and include this information on the Analysis Request Form. Other payment options include Visa or MasterCard. Please contact us if you have any questions about payment.

3. COMPLETE AND SIGN THE ANALYSIS REQUEST FORM

Click this Analysis Order Form link to load the form. The form is a PDF file.  Adobe^® Acrobat Reader^® is necessary to open the form.  It can be downloaded here    (download Adobe Acrobat)

Print the form and click the BACK button on your Browser to return to this page. Fill in the appropriate data concerning your sample, and send the form with your sample to:

Don’t forgot to include your Quote # to facilitate processing your sample.

4. SEND US YOUR SAMPLE

Samples can be sent USPS, FedEx or via any other carrier of your choice. Samples can also be dropped off in our lobby Monday – Friday from 8:00 A.M. – 4:45 P.M.

After hours, samples can be placed in our 24-hour drop off box located at the East Entrance of the building.

Please include any Material Safety Data Sheets (MSDS) if applicable as well as any precautionary measures that should be taken during sample handling and analysis. Samples should be packaged such that damage or contamination will not occur during shipping.

5. TEST RESULTS

You will receive a written report of the results upon completion. The report will include a description of the tests performed and discussion of the results. Reports can be emailed, faxed or mailed per your request. Analyzed samples will be returned to you or disposed of, per your request.
